looking for a compassionate and skilled Licensed Mental Health
Clinician to join our integrated Behavioral Health team. At our
FQHC, we believe that mental health is a fundamental part of
overall well-being. You will work alongside primary care providers,
psychiatrists, and care coordinators to provide high-quality,
accessible care to a diverse patient population in our Morrisville,
Vermont clinic. Role Summary The Licensed Mental Health Clinician
provides diagnostic assessments, crisis intervention, and
short-term or long-term evidence-based psychotherapy. This role is
central to our Integrated Care Model , where behavioral health is
embedded within primary care to address the "whole person." We are
looking for a clinician who is passionate about social justice,
health equity, and rural healthcare. Key ResponsibilitiesI.
Clinical Excellence Direct Therapy: Conduct individual, family, or
group therapy sessions utilizing evidence-based modalities (e.g.,
CBT, DBT, ACT, EMDR, or Solution-Focused Therapy). Comprehensive
Assessment: Perform psychosocial evaluations and diagnostic
assessments using DSM-5-TR criteria. Treatment Planning: Develop
and maintain person-centered treatment plans with measurable goals
and objectives. Crisis Intervention: Provide immediate assessment
and stabilization for patients in acute distress, coordinating with
local emergency services or hospitals when necessary. II.
Integrated Care & Consultation Warm Hand-offs: Respond to real-time
referrals from Primary Care Providers (PCPs) during medical visits
to provide immediate brief interventions or screenings.
Interdisciplinary Collaboration: Participate in daily huddles and
monthly case conferences with medical and psychiatric staff to
coordinate patient care. Consultation: Serve as a subject matter
expert for the medical team on behavioral health issues,
trauma-informed care, and socioeconomic barriers to health. III.
Documentation & Compliance Timely Charting: Maintain accurate and
timely clinical notes in the Electronic Health Record (EHR) in
accordance with FQHC and insurance standards. Compliance: Adhere to
all state and federal regulations, including HIPAA, mandated
reporting requirements, and professional ethical codes.
Qualifications Licensure: Must hold a current, active, and
unencumbered license in the State of Vermont as a Licensed Clinical
Mental Health Counselor (LCMHC) , or Licensed Drug and Alcohol
Counselor (LDAC) . (Master’s level clinicians close to licensure
may be considered for a "Candidate" role). Education: Master’s or
Doctoral degree in Counseling, or a related field from an
accredited institution. Experience: 2 years of post-degree clinical
experience is preferred. Experience in a community health, FQHC, or
integrated care setting is a significant plus. Skills: Proficiency
in trauma-informed care and experience working with substance use
disorders (SUD) or co-occurring disorders is highly desirable. The
