Job Description Job Description Agawam Rehab & Nursing Campus is seeking a caring LPN Licensed Practical Nurse to join our team f ull-t ime, part-time or PRN at our skilled nursing facility. Through strong personal relationships and thoughtful decision-making, each LPN Licensed Practical Nurse is entrusted to help support a meaningful life for, and with, the elders who are in our care. Responsibilities: Provide direct patient care. Conduct patient assessments and maintain accurate electronic health records (EHR). Administer medications and IVs as prescribed. Collaborate with healthcare teams to develop and implement care plans. Ensure compliance with HIPAA regulations and maintain patient confidentiality. Don't forget An active Massachusetts state LPN license is required. This is a direct hire position; we are not an agency.
