Job Description A skilled nursing and rehabilitation facility in Concord, MA is seeking a contract Physical Therapist. This role focuses on providing high-level care to residents within a 135-bed facility, with potential for full-time conversion. About the Opportunity: Schedule: F ull-time or Part-time available, no per diem Hours: Consistent days and hours, flexible based on availability Setting: 135-bed skilled nursing and rehabilitation facility Responsibilities: Providing and directing resident rehabilitation care Evaluating resident needs and establishing treatment plans Coordinating supervision of treatment plans with staff Participating in resident screenings Providing feedback on clinical and rehabilitation issues Qualifications: Master's Degree in Physical Therapy Current, active Physical Therapist (PT) license within the state Prefer experience/knowledge in Older Adult rehabilitation Knowledge of Governmental and Regulatory guidelines High-level care provision skills Strong communication skills
