Apply today!About the Job:The Case Manager RN will awork primarily in the Emergency Department, and collaborate with other members of the health care team to identify appropriate utilization of resources and a safe and effective transition plan ensuring clinical and social determinants of health are met. With the patient, family/caregiver, and health care team, create a transition plan appropriate to the patient's needs and resources including community providers to ensure effective communication and collaboration with a successful transition plan.Schedule:You will work 36 hours per week, (3 12-hour shifts), and every other Saturday.What You'll Do:

• Assesses and identifies patients' needs during the discharge planning process -
• Provide resources and community referrals for continuity of care -
• Care coordination with families, providers, and clinical team -
• Follows Medicare conditions when needed
• Utilization Reviews -Who You Are:
  • Bachelor's degree in a healthcare-related field or equivalent experience -
    • Current BSN or commitment to achieving the degree in a specified amount of time
    • 3 - 5 years of case management-related health care experience. - -
    • CCM or ACM preferred
