About the RoleWe are seeking a highly skilled and experienced Lung Transplant Nurse Coordinator to join our team at Mass General Brigham. As a key member of our transplant program, you will be responsible for providing exceptional patient care and coordination services to patients undergoing lung transplant surgery.Key Responsibilities

• Coordinate patient care and services throughout the transplant process, including pre-transplant evaluation, post-transplant follow-up, and ongoing care.
• Collaborate with the transplant team, including physicians, nurses, and other healthcare professionals, to ensure seamless patient care.
• Develop and implement individualized care plans for patients, taking into account their unique needs and circumstances.
• Provide education and support to patients and their families, addressing their concerns and questions throughout the transplant process.
• Manage and coordinate patient testing and procedures, ensuring timely and efficient completion of necessary tasks.
• Communicate effectively with patients, families, and healthcare teams, using clear and concise language to ensure understanding and cooperation.
• Participate in multidisciplinary team meetings and contribute to the development of policies and procedures related to transplant care.Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum 2 years of experience in pulmonary, thoracic surgery, or transplant nursing, with a strong background in patient care coordination.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
