Job Summary:
Integrated Care is meant to provide short-term support and active
patient management which is goal oriented, time limited and targets
patient care progress. The Integrated Care Pharmacist will develop
strategies to improve the integration and coordination of care and
the use of medicines addressing the long term needs in a select
group of high risk patients improving their outcome. The Emerson
Hospital Pharmacy Department is committed to optimizing medication
management for all patients by ensuring patients and their
care-givers are able to manage the often complex medication regimes
in the longer term as they transition to post-acute and community
care. The Integrated Care Pharmacist will promote the safe use of
medicines across the acute care community interface to improve
patient health and decrease hospital readmission.Minimum
Qualifications:
Experience/Education: A professional level of knowledge generally
acquired through an advanced degree and three years hospital
Pharmacy experience as a registered Pharmacist and/or the
completion of a Residency in Hospital Pharmacy.
Licensure and/or Certification: MA Pharmacist license. Registration
as a Pharmacist in Massachusetts without restrictions is
required.Knowledge, Skills, and Abilities:
Interpersonal skills necessary to interact with patients,
physicians, and nurses, etc.
Knowledge in the state of the art of IV practices including fluids
and electrolytes, incompatibilities of IV mixtures, chemotherapy,
etc.
Level of knowledge of automation, sterile products formulation,
drug distribution and extemporaneous manufacturing generally
obtained through an advanced degree relating to Hospital Pharmacy
and/or completion of a Hospital Pharmacy Residency.
Knowledge of the intricacies of intravenous parenteral nutrition
and of the prescription, preparation and formulation of such
products.
Knowledge of the principles of quality assurance and quality
control of sterile products to assure production of accurate,
sterile, contamination-free products.
Communication skills required for concise dissemination of
information to patients, nurses and physicians
Administrative skills necessary to coordinate personnel and
pharmacy operations.
Analytical skills necessary to accurately assess patients and help
plan their drug therapy.
Teaching skills required for the preceptorship of Pharmacy
Students.
Excellent customer service skills.
Must be able to read, write, and communicate in English.
Computer skills required.
Ability to maintain confidential information.
Ability to set priorities which accurately reflect the relative
importance of job responsibilities.
Ability to work independently and follow-through on assignments
with minimal supervision.
Ability to read and understand and interpret current scientific
literature.
Knowledge of and ability to perform age specific and disease
specific pharmacokinetic calculations.
Ability to establish and maintain cooperative working relationships
with coworkers and professional staff.
Ability to effectively communicate orally on a one-to-one basis
using appropriate vocabulary and grammar to obtain information,
explain policies and procedures, and to persuade others to adopt a
specific opinion or action.
Ability to function as a Staff Pharmacist when needed.
