We are neighbors helping neighbors.What will you be doing:We are looking for an experienced, talented, and compassionate RN Case Manager for a full-time position for the Hull area.The RN Case Manager is responsible for providing direct patient care in the patient's home. Must have a strong clinical background to conduct assessments, develop care plans and work collaboratively with the patient's physician to provide optimum care in the home.
