Job Title: Registered Nurse - Case ManagementMass General Brigham is seeking a skilled Registered Nurse (RN) to join our Case Management team in Brookline, Massachusetts.About the Role:

• Employment Type: StaffAs a Registered Nurse in our Case Management department, you will play a critical role in enhancing the delivery of patient care services along the continuum of care. You will collaborate with the healthcare team to evaluate, predict, and facilitate the trajectory of patient care, ensuring the individual's health care needs are met efficiently and expeditiously.Responsibilities:
  • Evaluate and predict patient care needs
  • Collaborate with the healthcare team to develop and implement care plans
  • Coordinate patient care services along the continuum of care
  • Monitor and evaluate patient outcomes

    • RN Case Manager experience required
    • Be a licensed RN in the state of Massachusetts
    • Minimum of 2 years Med/Surg experience required
    • Case Manager certificate preferred
    • ADN required, but BSN strongly preferred
